Registration and Required Forms
Registration for Little Larks workshops, sessions, camps, and events is completed online. For your family’s privacy and security, required forms are not posted publicly on this website; they are provided securely as part of the registration process, after you begin enrolling your child. This page is a general overview and is pending final legal and licensing review.
How Registration Works
Once you start registration for a workshop, session, camp, or event, we send you the required forms for that specific program through our secure online registration and e-signature system. You will complete these forms before your child’s first session.
Required Forms & Documents
Depending on the program, required forms may include a registration and emergency contact form, a health and medication information form, a photo and media consent form, and our enrollment agreement and liability waiver. The exact forms required for each program depend on its structure, ages served, and licensing status, which we are finalizing with California Community Care Licensing.
Electronic Signatures & Records
Little Larks uses a secure third-party e-signature platform, such as DocuSign or HelloSign, to collect signatures on registration and enrollment forms. During registration, we record the guardian’s signature, the date and time it was submitted, and the version of each policy or form that was accepted, so we have an accurate, time-stamped record of what each family agreed to.
Updating Your Information
If your contact information, emergency contacts, or your child’s health information changes after registration, please let us know as soon as possible so we can update our records.
A Note on Licensing
Whether a specific Little Larks program requires a California child-care license, and which state forms apply, depends on the program’s structure, ages served, schedule, location, and supervision model. We are confirming this classification with California Community Care Licensing before opening enrollment for programs such as our winter camp, and our registration forms will be updated to reflect that review.
